GENERAL FINANCIAL ASSISTANCE PROGRAMS

QUEST CARD
Food assistance (aka food stamps) and cash assistance benefits provided by county programs are credited to an Electronic Benefit Transfer Card (QUEST CARD).  This card can be used like a credit card for purchasing groceries (look for QUEST CARD logo in the store window), and for withdrawing cash benefits from ATMs and some supermarket grocery stores.

The account status of a QUEST CARD can be learned by telephoning customer services (1.888.328.2656) or going online at www.coloquestcard.com

 FEDERAL. STATE & COUNTY PROGRAMS

AND (Aid to the Needy Disabled) provides financial assistance to individuals who are temporarily or permanently unable to work due to a disability.
REQUIREMENTS: proof of disability; proof of applying for SSI, and social security numbers of each person applying.

GA (General Assistance) Emergency financial assistance for utilities (when LEAP is not available), motel vouchers, and the remaining $100 of past due rent if an eviction notice or demand for payment has been served.  GA is also available for burial assistance.
ELIGIBILITY: elderly, disable or parent with children under 18 living in the home; resident of the county where application is being made; not receiving public assistance.

DIVERSION -  assists households for up to six months to avoid going or staying on public assistance
REQUIREMENTS: must be eligible for TANF, have roof of employment, and not be receiving benefits in any other county or state in the month of referral or application to this program.  After receiving Diversion, cannot reapply for benefits for two months after receiving Diversion benefits.

LOW INCOME CHILD CARE ASSISTANCE -
ELIGIBILITY: working parents or parents under 22 who are in high school or studying for a GED.  Benefits are based on family size and gross family income.  Benefits are available to qualifying families for children from birth through 12 years of age.  Eligibility is re-evaluated every six months.
REQUIREMENTS: birth certificate(s), social security numbers for household members; proof of household income; and proof of child immunizations.
SERVICES: parents receiving assistance can choose a child care provider; although the county may complete a background check on a non-licensed provider.  The provider is paid directly by the county.  Parent(s) may be required to co-pay part of costs based on a fee schedule.

TANF (Temporary Assistance to Needy Families) provides temporary cash assistance option for very low-income families with children under the age of 18  and for women who are at least 6 months pregnant.  Funds are subject to availability and a 60-monthly lifetime limit for participants.
REQUIREMENTS: proof of monthly income and value of family resources and earned income; proof of pregnancy (if applicable); immunization records of children; photo ID and social security numbers of all household members
